Hospitalists Services at Mount Sinai South Nassau

As a service to our patients and our physicians, Mount Sinai South Nassau employs a staff of full-time hospitalists to assist with in-hospital care. This allows our physicians to concentrate on seeing patients at their practices, while our hospitalists update and consult with them about their admitted patients. Our hospitalist team is made of primary care physicians who specialize in either family medicine or internal medicine.

What is a hospitalist?
A hospitalist is a physician who specializes in managing the clinical problems of acutely ill, hospitalized patients. Hospitalists manage your hospital care, coordinating and consulting with specialists, monitoring your progress, and staying in close contact with you and your family. Our Hospital Medicine team consists of Internal Medicine and Family Medicine trained physicians.

What are the day-to-day responsibilities of a hospitalist?
The hospitalist team helps co-manage patients with other departments and specialties as well as coordinate closely with case management for a safe discharge. Our hospitalists review and organize the care of patients during their stay in the hospital.

Why does Mount Sinai South Nassau choose to have hospitalists?
Hospitalists specialize in the care of the patient while they’re in the hospital. Our hospitalist physicians communicate with patients’ primary care providers who may not come to the hospital so that when the patients are ready to go home, all of their records are available for discussion with their primary care provider.

How does a hospitalist benefit patients?
Hospitalists are available day and night. Because the hospitalists are always in the hospital, they closely monitor you during your hospitalization, follow-up on test results, and continuously adjust your treatment regimen throughout the day and night. Hospitalists are accessible in cases of emergencies and whenever concerns arise. Hospitalists provide the highest quality care in a timely and personalized manner.

How often will my hospitalist see me?
Your hospitalist will see you during morning rounds and often return to discuss test results, treatment plans and communicate with family.

How will I receive updates from my hospitalist?
Updates are usually done in person at the patient’s bedside. Occasionally, your hospitalist may call you to discuss your treatment plan in addition to morning rounds.

Will my hospitalist be with me throughout my admission?
Your hospitalist will be with you always from admission through discharge. Depending on their work schedule, another hospitalist may take over your care after a thorough handoff and discussion of your treatment plan.

How do hospitalists handle transitions of care when a patient is ready to be discharged?
The hospitalist team meets with case management on a daily basis to determine the best and safest discharge plan for the patient and their family.

Dmitry Konsky, DO, MBA, FACP
Mount Sinai South Nassau’s Director of Hospital Medicine, Dmitry Konsky, DO, is board certified in Internal Medicine.  He earned his medical degree from NYIT College Of Osteopathic Medicine.  He completed his residency in Internal Medicine at Maimonides Medical Center in Brooklyn, NY.  He also completed his executive MBA at Adelphi University.

Dr. Konsky is a fellow of the American College of Physicians and a member of the American Medical Association and Society of Hospital Medicine. His specialty includes inpatient care, medical operations, quality, patient safety and healthcare information technology.
