Outpatient

Outpatient services are those that, while not requiring a hospital admission, are not usually performed in a doctor’s office, including:

  • Ambulatory surgery
  • Outpatient services which are preventive, diagnostic, therapeutic/rehabilitative or palliative
  • Referred services such as radiology (x-ray, MRI, bone density) or laboratory work (blood tests, etc.)
  • Emergency room
  • Mental Health Services such as Outpatient Mental Health Clinic (516-546-1370), Partial Hospitalization Program (516-546-6924) and Intensive Outpatient Program (516-546-6924)
Whatever service you may require, here are some general suggestions to help make your admission process run as smoothly as possible.

What to bring with you

  • Any reports your physician gave you to bring to the hospital
  • Relevant medical insurance information, including insurance cards, pre-certification, and any other documentation required by your insurer
  • Your photo ID, such as a driver's license or any other form of picture ID and proof of address such as a utility bill
  • Your Social Security card
  • A list of telephone numbers where members of your immediate family can be reached if necessary
  • Completed Medicare Secondary Payor Questionnaire, if you are entitled to Medicare benefits
  • Any copay, coinsurance, deductible or self pay deposit

What to leave home
It’s best that you not bring any valuables with you, such as jewelry, money, etc. Any valuables, however, can be held by the Hospital and claimed by the patient or the next of kin from the cashier’s office. No valuable will be released without proper identification.

Take care of any pre-op testing or other requirements

When you come in for your pre-op testing, please make sure to bring:

  • Relevant medical insurance information, including insurance cards, pre-certification, and any other documentation required by your insurer
  • Your photo ID, such as a driver's license or any other form of picture id.

If you are not sure if we participate with your insurer, please call your insurer for confirmation – and for any special considerations. For example, most patients are required to obtain their Lab and Radiology services at a dedicated facility. Your insurer may require you to pay for any services provided that is not covered by your contract.

If you do not have insurance, or are unable to pay for the hospital services provided, our Financial Assistance Service may be able to help.

Understand your rights as a patient
As a patient in a New York State hospital, you have certain rights and protections guaranteed by state and federal laws. These rights exist to help ensure the quality and safety of your hospital care.

To help you understand these rights, the New York State Department of Health and its Consumer Health Information Council published a booklet "Your Rights as a Hospital Patient." It will be provided to you upon your admission to the hospital. However, if you would like a copy of this booklet before your admission, please call the Admitting Office at 516-632-3990.
